متطلبات السفر الإلزامية قبل السفر إلى الفلبين

إن من متطلبات السفر الإلزامية لمن يرغب السفر إلى الفلبين هو الحصول على بارك كود التسجيل في مركز الإفصاح الطبي الفلبيني
وذلك باتباع الخطوات التالية:

1- تعبئة النموذج التالي ( سيظهر لك فيديو ارشادي انتظر دقيقة وبعدها سيظهر لك SKIP & NEXT اضغط على هذا الاختيار للوصول للنموذج )

2- بعد تعبئة النموذج أعلاه سيتم ارسال رقم التسجيل الى الإيميل المسجل في النموذج.
3- نسخ رقم التسجيل من الإيميل ثم تسجيل الدخول في الموقع التالي بإستخدام إسم العائلة ورقم التسجيل:

4- إكمال الخطوات حتى مرحلة الحصول على بار كود الإفصاح الطبي
5- طباعة النموذج لتقديمه لموظف المطار عند كاونتر انهاء اجراءات السفر
6- الحصول على فحص PCR سلبي قبل السفر بـ 48 ساعة
7- تجديد العقد من السفارة الفلبينية او القنصليات ولن يسمح بعودة العاملة بدون تجديد عقد العمل.
يمكن للعاملة السفر, ثم يتم تجديد العقد بدون حضورها وارسال الاصل عبر شركات الشحن.

تنبيه
قد لا يُطلب من المسافرين الدوليين الوافدين الالتزام بالحجر الصحي الإلزامي في المنشأة وفقًا لقرار IATF رقم 159 ق. 2022 ، إذا:
1. كان الراكب قادرًا على تقديم اختبار PCR سلبي - تفاعل البوليميراز المتسلسل (RT-PCR) الذي تم إجراؤه في غضون ثمانية وأربعين (48) ساعة ، أو اختبار مستضد مختبري سلبي تم إجراؤه في غضون أربع وعشرين (24) ساعة ، قبل المغادرة من بلد المنشأ.
2. أو إذا تم تطعيمه بالكامل ويجب أن يكون لديه الأدلة التالية على التطعيم ضد COVID-19 المعترف به بموجب لوائح IATF الحالية:
أ. الشهادات الدولية لمنظمة الصحة العالمية في التطعيم والوقاية ؛
ب. VaxCertPH
ج. أو شهادة رقمية وطنية / حكومية من الحكومة الأجنبية التي قبلت VaxCertPH بموجب ترتيب متبادل ما لم يسمح IATF بخلاف ذلك ( المقصود شهادة تثبت الحصول على الجرعات يتم تحيملها من تطبيق توكلنا )

International arriving passengers may no longer required to observe mandatory facility-based quarantine as per IATF Resolution No. 159 s. 2022, if:
1. Passenger is able to present a negative Reverse Transcription - Polymerase Chain Reaction (RT-PCR) test taken within forty-eight (48) hours, or a negative laboratory based Antigen test taken within twenty-four (24) hours, prior to departure from the country of origin.
2. Fully Vaccinated and must have the following proofs of vaccination against COVID-19 recognized under existing IATF regulations:
a. World Health Organization International Certificates of Vaccination and Prophylaxis;
b. VaxCertPH; or
c. National/state digital certificate of the foreign government which has accepted VaxCertPH under a reciprocal arrangement unless otherwise permitted by the IATF


Isa sa mga mandatoryong kinakailangan sa paglalakbay para sa mga gustong bumiyahe sa Pilipinas ay kumuha ng registration park code sa Philippine Medical Disclosure Center
Sa pamamagitan ng pagsunod sa mga hakbang na ito:

1- Punan ang sumusunod na form (Lalabas ang isang gabay na video, maghintay ng isang minuto, pagkatapos ay lalabas ang SKIP & NEXT, i-click ang opsyong ito para ma-access ang form)

2- Matapos punan ang form sa itaas, ang numero ng pagpaparehistro ay ipapadala sa email na nakarehistro sa form.
3- Kopyahin ang numero ng pagpaparehistro mula sa email at pagkatapos ay mag-log in sa sumusunod na website gamit ang apelyido at numero ng pagpaparehistro:

4- Kumpletuhin ang mga hakbang hanggang sa yugto ng pagkuha ng medikal na disclosure bar code
5- Pagpi-print ng form na isusumite sa empleyado ng paliparan sa check-in counter
6- Pagkuha ng negatibong PCR test 48 oras bago maglakbay
7- I-renew ang kontrata mula sa embahada o konsulado ng Pilipinas, at ang balik manggagawa ay hindi papayagang bumalik nang hindi nagre-renew ng kontrata sa pagtatrabaho.
Maaaring maglakbay ang manggagawa, pagkatapos ay i-renew ang kontrata nang wala ang kanyang presensya at ang orihinal ay ipinadala sa pamamagitan ng mga kumpanya ng pagpapadala.

One of the mandatory travel requirements for those wishing to travel to the Philippines is to obtain a registration park code at the Philippine Medical Disclosure Center
By following these steps:

1- Fill out the following form (A guide video will appear, wait a minute, then SKIP & NEXT will appear, click on this option to access the form)

2- After filling out the above form, the registration number will be sent to the email registered in the form.
3- Copy the registration number from the email and then log in to the following website using the last name and registration number:

4- Complete the steps up to the stage of obtaining the medical disclosure bar code
5- Printing the form to be submitted to the airport employee at the check-in counter
6- Obtaining a negative PCR test 48 hours before travel
7- Renew the contract from the Philippine embassy or consulates, and the worker will not be allowed to return without renewing the employment contract.
The worker can travel, then the contract is renewed without her presence and the original is sent through the shipping companies.
